Medicaid Waiver Service Facilitator/Case Manager (Independent Contractor – 1099) - Hampton Roads, Tidewater.
The Service Facilitator/Case Manager (SF) is responsible for coordinating and monitoring Medicaid Home and Community-Based Waiver services to help individuals safely remain in their homes and communities. This position is 100% remote. The SF will conduct in-home visits for initial intake assessments, develop and update person-centered Plans of Care, and complete required routine and reassessment visits in compliance with Virginia DMAS regulations.
The role requires maintaining accurate documentation; strong organizational, communication, and time-management skills are essential, as this position requires working independently and managing a flexible schedule. Must have reliable transportation and the ability to travel throughout the surrounding counties.

Job Requirements & Education:
- High school diploma or GED required.
- Experience working with individuals with chronic health issues, intellectual/developmental disabilities, or mental health disabilities required.
- Successful completion of Virginia DMAS Service Facilitation Module Training (before providing services).
- Knowledge of Virginia Medicaid Waiver programs and person-centered planning.
- Excellent documentation, computer, communication, and organizational skills.
- Valid driver's license, reliable transportation.
- Ability to pass all required background screenings.
Preferred:
- Bachelor's degree in Human Services, Social Work, Psychology, Rehabilitation Services, Disability Services, Nursing, or a related field.
- Experience working with individuals with intellectual or developmental disabilities.
- Experience with Virginia Medicaid waiver programs and forms.
- Experience with Consumer-Directed service and Fiscal Agent processes.
- Knowledge of DMAS and DBHDS regulations and documentation requirements.
- Experience with electronic health records or case management/documentation systems.
